Using Our Prepared Small Claims Filing Forms

The forms we make available on this site have been prepared for your use in pursuing the Pompano Beach Flying Club in Small Claims Court as described on the “Filing Your Own Small Claims Case” web page.

The forms have been pre-completed with all necessary information related to the Club. There are only six boxes on the first page of the document that you need to fill in before printing three copies.

PAGE 1: STATEMENT OF CLAIM

  1. PLAINTIFF(S): Enter your full name (with middle name). If more than one plaintiff is involved in your particular dispute, include the full names of all plaintiffs in this box.
  2. Description: Although you can try to squeeze all the details into the next box, it will be easier for you to prepare a separate document to attach to your filing. In that case, “SEE ATTACHED” is appropriate here, because the details of your Complaint will be attached on separate pages.
  3. Demand Amount: This is important. Although this form is pre-completed with a $1,700 amount, you should carefully calculate the total of what you believe the Club owes you and enter it here. Do not enter court or other “costs” you incurred in the pursuit of your case. If you prevail, court and other costs can be added to the judgment, but you do not need to anticipate that here. Only enter the amount you are owed.
  4. PLAINTIFF OR ATTORNEY: This should be just one name, your full name as lead plaintiff.
  5. ADDRESS OF PLAINTIFF/ATTORNEY: Enter your complete mailing address here. Until you create an E-Filing Portal account, all court documents will be mailed to this address.
  6. PLAINTIFF/ATTORNEY TELEPHONE: Enter your telephone number (just the number without notation as to whether it is home, mobile, etc.).

PAGE 3: SUMMONS TO APPEAR

For our prepared forms, the information you entered on page 1 (the Statement of Claim) will automatically appear in the three boxes on this form. CAUTION: If you change any information on this page, it will also change what appears on the first page. Generally, you don’t need to do anything but complete the first page, but double check the forms before heading to the Courthouse for filing. If all else fails, these forms are available at the Courthouse and can be filled in by hand.
